Appearance of a deep rooted boil without any opening within the two fingers around the anus with pain and fever is known as Bhagandara-Piḍakā (anorectal abscess). When this ano-rectal abscess breaks through the perineum, anus or bladder and have an opening then it is called Bhagandara (fistula-in-ano).

Signs and Symptoms


Itching, pain in the waist, burning sensation and inflammation near the anus are the symptoms and signs which patient has before the appearing of Bhagandara. After forming Bhagandara intense pain, oozing of reddish frothy discharge from the channels are the main signs and symptoms.

Treatment


Generally the disease required surgical intervention. Application of Kṣāra Sūtra by an expert is very successful and chances of recurrence are rare, As soon as the swelling appears care should be taken that pus does not form. After the formation of pus, it should be treated as Vraṇa. Following oral preparations are useful if taken along with the application of Kṣāra Sūtra or surgical treatment.

Single Drugs


1. Kṛṣṇa Tila (black sesame) – 12 to 24 g. to be taken with 100 to 250 ml. milk thrice a day.

Simple Preparations


1. Decoction of wood of Khadira (khair) as much as possible for quenching thirst.

2. Decoction of equal part of fruit rinds of Harītakī (chebulic myrobalan), Vibhītaka (balerie myrobalan) and Āmalakī (embelic myrobalan), Śuddha Guggulu, Viḍaṅga, Śuddha Lākṣa (purified lac) – 14 to 28 ml. to be taken thrice a day.

3. Take Triphalā – 12 g., Śuddha Guggulu – 60 g. and Ghṛta (ghee) 10 g. and make pills of 1 g.; one pill is to be taken twice a day with warm water.

Compound Preparations


1. Pañca Tikta Ghṛta: 12 to 24 g. to be taken with 100 to 250 ml. milk thrice a day.

External Applications


1. Wash the ulcer with Triphalā Kvātha.

2. Apply Malhara prepared from dried Sarala Niryāsa in Ghṛta (ghee) on the ulcer.

3. Daśāṁga Lepa in Ghṛta (ghee) to be applied on the abscess.

4. Jātyādi Taila for Varti.

5. Lepa of Tila (sesame seed), leaf of Nimba, root of Yaṣṭimadhu (glyyrrhiza) prepared in Dugdha (milk), to be applied on the ulcer.

6. Prepare lepa of equal part of root bark of Śveta Trivṛt (white turpeth), root of Dantī (wild croton), Tila (sesame seed), in water and apply on the ulcer.

7. Lepoa of Kalka Of Rasāñjana, rhizome of Haridrā (turmeric), bark of Dāruharidrā (barberry), root of Mañjiṣṭhā (madder), leaf of Nimba, leaf of Teja Bāla (toothache tree), root bark of Trivṛt (turpeth), Cavya and root of Dantī (wild croton), to be applied on the ulcer twice a day.

Pathya


Mudga (phaseolus bean); leaf of Vāstūka (a variety of chenopodium) and other leafy vegetables, light foods are useful for the patient of Bhagandara.

Apathya


Incompatible items of food in diet, too frequent meals, irregular gaps in meals, exercise and riding are harmful for the patient of Bhagandara (fistula-in-ano).
